Job Description
Performs assigned activities, implements assigned parts of the Individual Support Plan (ISP), assists clients in learning daily living skills, participates in recreational and social activities, accompanies individuals to appointments, if needed, and observes/documents clients behavior. Provides training and direction to individuals in the·area of personal care, daily living skills, employment skills, and recreational activities depending on the individual's needs. Observes and maintains an awareness of the individual's behavior and physical whereabouts to ensure their safety. Documents person centered summary of activity in Carelogic (Electronic Medical Record computer program) daily. Noting progress or lack of progress on individual goals. Provide transportation for individuals to appointments and activities, if needed. Participates in Individual Support Plans (ISP). Participates in providing safe, secure, clean and attractive environment for individuals, visitors and staff. Attends and completes all required training. Must be Dependable and Flexible.
 
Minimum Qualifications
High School Diploma or GED.  One year of experience  providing social or direct care to individuals with developmental disabilities - 0 R an associate's degree in a related field.  Must have a valid driver's license and a good driving record. Must be computer literate.

Preferred Qualifications
Experience in documentation through electronic medical records.
